    Dear Community Partners:
     
    Kaiser Permanente South Bay is pleased to announce that we are rolling-out a new online application system which we call Mosaic. We are confident that Mosaic will ensure a more user-friendly and efficient experience for our applicants.

    Application deadline is Friday, December 21, 2018 by 5:00PM PST
     
    To access Mosaic and our funding opportunities, you will need to create a new account by clicking the Create Account button on the login page. Please note that if you have an account with our old system, that account does not carry over. Creating an account in Mosaic is a two-step authentication process which helps to ensure that you receive future useful notifications from our system.
     
    We strongly encourage you to review the attached Mosaic User Guide for Sponsorship Applicants before starting a sponsorship application. This guide has been developed to help you navigate within Mosaic and offers clarification on application questions as well.
